​What if someone gives me Cash or Checks for a Calendar?

If you need to submit a check or cash to purchase calendars it MUST be accompanied by this form. 

Each purchaser needs a unique form and each form can accommodate 12 calendars. If additional calendars are needed you may submit two forms. You will need the email of the recipient so they receive their calendar AND the address so the check can be mailed to them in the instance they win!

You will NOT receive credit for these sales until both the Cash/Check and COMPLETED Form are returned to  Mrs. Shaw.
