This form is to be used EVERY TIME you receive a check, cash, or pledge donation. Fill out the entire sheet and send a picture to Clint Beilke at clint.beilke@balladhealth.org. We will then upload the donation to your individual or team fundraising page within 24 hours to reflect your most accurate fundraising total. Please email Clint to pick up cash or checks.
