Hospice of Cincinnati Has a New Look!

INTRODUCING: HOC NAVIGATORS

Hospice of Cincinnati has unveiled a new design, a new brand, and a new way of looking at care for advanced illness and end-of-life.

As of Aug. 1, we’ve rebranded our growing array of services. While the Hospice of Cincinnati name and service remains the same, all of our programs (including Hospice of Cincinnati) now fall under a new “umbrella” name – HOC Navigators.

The rebranding includes a new look for our organization and our programs, which have grown beyond end-of-life care to include services for people with serious and advanced illnesses who may not be ready for hospice care. The name “HOC Navigators” and the accompanying compass logo reflect our commitment to guiding people through advanced illness and end-of-life, wherever they may fall on that spectrum.

Hospice of Cincinnati

End-of-life care for those with a life expectancy of 6 months or less; according to goals, wishes and preferences; wherever the patient calls home.

A specialized care team of a Hospice and Palliative Certified Physician and Nurse Practitioner, RN, LPN, STNA, SW, Chaplain and Volunteer in a patient and family-centered model.

  • ​4 Inpatient Care Centers for crisis symptom management and Respite Care
  • Continuous Home Care and 24/7 in-person or telephonic RN support for crisis

Guiding you through advanced illness and end of life. One's advanced illness trajectory isn't linear. HOC programs meet patients where they are.

Advanced Illness Programs:

  • Social Connections 
    • Loneliness and social isolation are not only fatal, but lead to higher level of co-morbidities.
  • PalliaCare Cincinnati 
    • Home-based palliative care provides an extra layer of support for those with serious illness regardless of life expectancy or prognosis and focuses on short-term stabilization of symptoms.
  • Hospital-Based and Clinic-Based Palliative Medicine
    • Palliative consult in the hospital and outpatient care settings in collaboration with acute care hospitalists, disease specialists and primary care physicians.
  • Conversations of a Lifetime
    • Conversations of a Lifetime is an initiative by Hospice of Cincinnati to transform end-of-life care in Greater Cincinnati and improve end-of-life experiences. The program encourages earlier end-of-life planning conversations between physicians, patients and families.

Grief Services

The Goldstein Family Grief Center

Promoting healing for life after loss by supporting individuals and families through grief. We believe the death of a loved one can be a meaningful experience with the support and guidance of grief counselors. We provide free grief counseling for 13 months after the loss of a loved one.
