Bayshore’s Carepath – accessed through your private insurance carrier, provides Bayshore’s Nurse Navigators (your single point of contact) provide a clearly defined path; allowing you to relax and focus on other aspects of your life. All services provided are delivered by experienced and accredited oncology nurses and directed by a medical advisory board of Canada’s leading Oncologists. Bayshore’s Nurse Navigators explain tests, results, and treatment options to help make best- informed decisions.
Home Health – Personal Support
Whatever the reason, whenever the need – Bayshore Home Health provides around the clock care and support. From initial diagnosis, Bayshore is engaged to provide Every Day Care relieving stress and physical impact to the people we serve; providing personal care (hygiene and grooming), performing light housekeeping, shopping, meal preparation, attending appointments and running errands. If a hospital stay is required Bayshore’s Enriched Care Nurses are available for in-hospital personal care assistance, explanation of tests and results, and to provide help home-from-hospital (includes medication pick-up and home safety assessments). If out-of-hospital day treatments are part of the care plan, Bayshore’s Essential Care Program provides knowledgeable Nurses, able to provide additional support through medication reminders and delivery, and any additional personal care required not conducted by our Every Day Care caregivers. Essential Care Nurses provide peace of mind to those requiring palliative care and are available as needed.
Bayshore’s Specialty Pharma provides patient centered care with oncology, neurology, rheumatology and rare disease experts - delivering patient care from diagnosis to disease progression through pharmacy, reimbursement services, case management, therapy administration and Nursing Services. Bayshore’s Specialty Pharma offers Patient Assistant programs including Nurse case management, Call center service, Reimbursement investigations , PV and data. Bayshore has a National in-house Pharmacy network with Warehousing and 3pl distribution to ensure timely access to required mediations. Through national initiatives Bayshore’s Specialty Pharma is proud to offer Specialty Nursing in our private clinics or from the safety and comfort of your own home. Bayshore’s Specialty Pharma is proudly part of the Preferred Pharmacy Network and have been recognized for Health Case Management.
Bayshore has a network of over 90 Infusion Clinics, employing more than 3500 Nurses who provide services safely in one of our specialty clinics or from comfort of your own home. Enabling patient access to therapy is just the beginning - Bayshore’s Infusion Clinics: Provide patient support programs designed with pharmaceutical manufacturers, injections and infusions either through PSP [AB2] or as a referral, are partners in clinical trials working closely with primary investigators and their study sites across Canada, and a participate in partnership programs customized and designed to address local or national concerns with Healthcare teams like yours.
